Family Income Survey
This form needs to be completed annually. If there are multiple students attending the same YES Prep campus, families only need to submit one form.
Records Request Form
All students and families requesting any form of records like transcripts, verification of enrollment, report cards etc., must submit a Records Request form to the Campus Registrar. Records will be delivered within 24-48 hours of submission.
Change of Address Form
We strive to always have the most up to date information for our families. If you have a change in your address, please turn in this form to the Front Office in order to update your information in our system.
Medication/Treatment Administration Consent Form
If your child's doctor prescribed a medication that needs to be administered by our Certified Medical Assistant, parents are required to turn in the medication along with this form to the CMA in order for the administration of medication to occur.
Incident Report Form
If you have concerns about your child and what is happening on campus, such as, bullying, fighting, stolen property, etc. Please fill out our Incident Report Form. Our Student Support Team works diligently to investigate and ensure every student's safety on campus.
